The Short Term Experiences of Constrained Acetabular Component for Unstable THA

Abstract
Purpose: We describe our short term clinical experience and make recommendation with regard to the appropriate use of constrained acepabular components for an unstable THA(ed note: define THA.).

Materials and Methods: Between April 2000 and December 2003, six S-ROM constrained acetabular components were used in a revision total hip arthoplasty. The average age at surgery was 61 years (52~82 years) old. The average follow up was 27 months (range 17~39 months). Four cases were a recurrent dislocation after a total hip arthroplasty and two cases were an intraoperative instability during revision arthroplasty.

Results: There were no additional dislocations after revision THA during the follow up. One patient died after 2 years but there was no instability before death. One hip required a resection arthroplasty as a result of an infection. At this short term follow up, there was no evidence of femoral or acetabular component loosening on the radiograph.

Conclusion: The judicious use of this constrained acetabular component as a salvage measure for desperate cases of hip instability during or after total hip arthroplasty is recommended. However, it should not be considered as a first treatment option for patients, who have a routine instability after a THA because of the possible increased polyethylene wear and increased interfacial stresses.
